We’re looking for a Field Relations Coordinator, someone who’s ready to grow with our company. In this position, you’ll be responsible for deployment of team-based population management including developing and maintaining positive relationships with physicians, members and their families, and providers. You’ll also be responsible for screenings, performing care plan interventions when licensure is not required, coordinating activities, referral and linkage to community resources, monitoring and tracking outcomes.

Job Duties & Responsibilities 
•    Develop and maintain satisfactory business relationships with participating providers 
•    Collaborate with Service Coordinator, Care/Case Manager, and other members of the care coordination team and providers to perform care plan interventions where able and monitors care plan outcomes and member needs 
•    May act as the lead service coordinator for STAR Kids Level of Care 3 Members who are not receiving LTSS/PDN services with the support of a Care/Case Manager to perform assessments and build member care plan 
•    Review and interpret all relevant information with Member/family and develops/supports the initiation of a care plan and self-management plan with family 
•    Identify options and initiate collaboration with referral sources to ensure Member/family linkage to specialists, homecare providers, financial assistance, housing, meals, transportation, and other community resources as appropriate for Members with unmet needs 
•    May travel to member homes for periodic screening of unmet needs to support educational needs and goal attainment seeking Member/family input in care plan 
•    Monitor service delivery to members including satisfaction and quality of services 
•    Deliver exceptional customer service to contracted providers and Members 
•    Ensure thorough communication and education to Members/providers an understanding of delivery system capabilities to improve access and address Member needs 

Skills & Requirements 
•    H.S. Diploma or GED required 
•    Bachelor's degree preferred 
•    Driver’s license in any state required, and auto insurance required 
•    CHW – Community Health Worker certification by Texas Department of State Health Services (DSHS) preferred 
•    5 years of experience in healthcare, local and state programs for children with disabilities or managed healthcare setting required 
•    A bachelor's degree may substitute for four (4) years of required work experience 
•    A master's degree may substitute for five (5) years of required work experience 
•    Authorization to Release Personal Information must be signed prior to beginning work in this role. As a remote worker/road warrior with the company Health Plan, this consent allows the company to disclose an employee’s home address for the purposes of geomapping
