We are searching for a Supervisor Community Health- Someone to oversee the Field Relations Coordinators (FRCs) performing member outreach to improve member wellness at the Health Plan. Ensure productivity of the FRC team, appropriate documentation, and member satisfaction. Handle customer concerns and issues that require a high degree of appropriate knowledge, creativity, research and communication with internal and external resources. Assist leadership by developing relationships with community organizations and furthering the social determinants of health strategy.

Qualifications:
- Bachelor's Degree required, BS degree in health services, public health, or other health care relevant field preferred.
- 3 years of population health, community public health, ambulatory care, other clinical care, and/or healthcare admin experience required.
Responsibilities:
- Supervise and track Social Determinants of Health (SDOH) interventions
- Supervise the Field Relations Coordinator staff
- Provide excellent customer service to Internal and external customers
- Perform FRC Responsibilities, as needed
- Supervise daily operations
- Ensure timely and effective two-way communication of strategic and operational messages to teams.
- Manage the implementation of change initiatives while maintaining service levels and the highest levels of employee morale and productivity
