We are searching for a Care Coordinator with the Outpatient Complex Care Clinic — someone who works well in a fast-paced setting. In this position, you will provide a collaborative approach to assess, plan, implement, monitor, and evaluate the options and services required to meet an individual's health needs. This role provides comprehensive, ongoing case management services to patients by coordinating and managing the care of medically fragile and/or rising/high-risk patients while meeting service needs. As a care coordinator, you’ll partner with physicians to establish care and allocate resources associated with the patient's risk assessment.

Qualifications

  • Bachelor's Degree in nursing Or Associates Degree in nursing with acceptance or current enrollment in a BSN program and signed agreement Or Technical diploma in nursing with acceptance or current enrollment in a BSN program and signed agreement Or Must be currently enrolled in a nursing bridge program working toward an MSN.
  • RN - Registered Nurses License from the State of Texas required
  • BLS - Basic Life Support Certificate from the American Heart Association required
  • 3 years of pediatric nursing experience required
  • Experience in community health, complex pediatric patient care, home care, case management, managed care, or utilization review preferred

Responsibilities

  • Proactively identifies patients in need of care coordination through clinical rounds, medical management staff referrals, medical director/PCP, parents, staff, home care staff, and/or other internal areas via utilization of stratification tools
  • Assists the patient/patient's family in coping with illness by optimizing the patient's/family's self-care abilities and supporting their consumer rights
  • Performs a comprehensive goal-focused and individualized assessment for patients referred for evaluation, to include patient/family’s biophysical, psychosocial, environmental, discharge planning needs, and/or financial status
  • Identifies problems, barriers, and opportunities for intervention while proactively ensuring patient care needs are addressed
  • Provides resolution and revision of plans for any ongoing projects and assures adherence to the best practice techniques and utilization of evidence-based practice
  • Routinely assess patient’s status and progress; if progress is static or regressive, determine the reason, and proactively encourage appropriate adjustments in the care plan, providers and/or services to promote better outcomes
  • Serves as a communication link between inpatient and outpatient services as well as across the various health professionals involved in an individual patient’s care
  • Solves problems, identifies appropriate resources, adapt to ongoing change, and handle conflict management with a professional and supportive attitude
  • Stays abreast of professional credentials, training, clinical skills, and current health care developments relative to the population served to provide safe, quality nurse nursing care
