We are searching for a Supervisor of Care Coordination. Someone who will supervise front line staff by directing the activities for the team. Handles customer concerns and issues that require a high degree of appropriate knowledge, creativity, research and communication with internal and external resources. To assist with the management of incoming call traffic. To ensure resources are utilized efficiently, in accordance with program objectives, key performance indicators and in ways that consistently promote customer quality and satisfaction. To lessen the probability of customer complaints by managing problems referred by Member Service Representatives or other staff.

Qualifications:

  • Bachelor's Degree Licenses and Certifications Required
  • RN - License-Registered Nurses Texas Board or Nursing or Nurse Licensure Compact Experience required
  • 1 year Managed care experience required
  • 3 years' experience in population health, community public health, ambulatory care, or healthcare administration required

Responsibilities:

  • Supports the care management, population health, and/or behavioral health daily operations. 
  • Assesses, develops, implements and monitors a comprehensive plan of care through an interdisciplinary team process in conjunction with the member/family in internal and external settings. 
  • Performs all necessary communication and documentation functions
  • Ensures Team provides case management, discharge and service planning review for TCHP members including assessment, planning, implementation and evaluation of educational, Long Term Services & Supports (LTSS), and social service related options
  • Conducts initial and on-going system training for Case Management staff.
  • Participates in quality initiatives and performance management.
  • Supervision of daily operations.
