We are searching for a Patient Navigator with our  Outpatient Care Coordination Clinic– someone who works well in a fast-paced setting. In this position, you will provide support for the coordination of patient care activities and assist patients navigate through all the steps in obtaining the necessary and appropriate care. Additionally, you will ensure a seamless transition from inpatient to the post-discharge follow-up in the outpatient clinic, liaise with the patients/families and required outpatient services, and assist the patient with coping with their illness.

Qualifications

  • High school diploma or GED
  • 1 year experience in a health care setting, including being a personal caregiver or in home health
  • Bilingual in Spanish and English preferred
  • Experience with EPIC highly preferred
  • Experience working within a clinic setting and/or home health setting preferred

Responsibilities

  • Develop a relationship with the patient/their family prior to discharge and facilitate the inpatient-to-outpatient transition, communicate with the patient/family during initial discharge period, and track the care coordination needs while they are in transition
  • Interview and obtain data about the patient’s progress towards their goals and work with the patient to modify an individualize the care plan (if necessary)
  • Serve as the primary source of contact for paneled patients and support the patient hand-off to the multidisciplinary team
  • Schedule appointments, assist with the coordination of medical transportation, send prescriptions, and assist with DME requests
  • Collaborate with key stakeholders to assist with the coordination of activities
  • Maintain all assigned databases to ensure data is complete and accurate
  • Greet new patients and present new patient educational materials, and ensure proper patient understanding of all services offered
  • Provide assistance to the care coordinator by partnering with community organizations involved in the interests of the pediatric population and liaise with patients regarding their access to their health team members
  • Monitor and report patient/caregiver resource utilization, past and present treatment, and established goals (short- and long-term)
  • Maintain confidentiality concerning patient/family care issues
