We are searching for a Clinical Pharmacy Specialist — someone who works well in a fast-paced setting. In this position, you will manage and promote activities appropriate to ensure the medication use process, serve as mentor to pharmacists who provide services to the medical specialty, and assist in direct patient care activities to promote optimal drug therapy and resolve medication use problems in a manner developmentally appropriate for the ages of patients served (newborn to adult).
Job Duties & Responsibilities
- Maintain punctual attendance consistent with Texas Children’s Hospital policies, regulatory, and federal, state, and local standards
- Participate in the clinical on-call program
- Communicate and collaborate effectively and appropriately with other members of health care disciplines to provide comprehensive care to patients
- Communicate and treat all coworkers with respect and courtesy, avoiding loud and disruptive conversation, and/or actions and remarks indicating disrespect
- Attend and actively contribute to daily patient care rounds, clinical services meetings, hospital committees, task forces, and projects as assigned
- Utilize practice knowledge and skills to serve as clinical practitioner
- Participate in multi-disciplinary rounds with Heart Center service line, including cardiac intensive care and acute care cardiology
- Perform pharmacokinetic monitoring as indicated and discuss recommendations with appropriate medical professionals to ensure individualized patient care
- Monitor therapy for interactions and discuss with appropriate health care professionals to help ensure patient safety
- Serve as a member of patient care committees for designated service line
- Participate in quality improvement initiatives to reduce variation, achieve predictable results, and improve outcomes for patients
- Participate in the development of evidence-based clinical guidelines, protocols, and order sets to ensure appropriate provision of medical therapy
- Serves as a resource to pharmacy staff, physicians, and nurses
- Effectively plan and manage resources for continuity of care between the patient care area, pharmacy, and other settings
- Continually improve medication use processes and systems
- Oversee coordination of activities to ensure proper delivery of drug therapy
- Create and maintain policies and procedures affecting areas of responsibility
- Promote the use of the formulary and follow procedures to obtain non-formulary medications
- Identify and suggest agents to be added, deleted, or modified to/within the formulary and database
- Investigate, follow-up, and take appropriate action for medication incidents and adverse drug reactions
- Participate in the education of patients, caregivers, pharmacists, and pharmacy learners
- Provide medication education to patients/parents/caregivers
- Participate in the education and training of pharmacy employees, pharmacy students, pharmacy residents, and non-pharmacy personnel
- Develop educational programs, both service line specific and general education for pharmacists and interdisciplinary team
- Serve as a preceptor to pharmacists, pharmacy residents, and students
- Develop and implement clinical competency and education modules for staff
Skills & Requirements
- Being fully vaccinated against COVID-19, including any booster dose(s) of the COVID-19 vaccine recommended by the Centers for Disease Control when eligible, is required for all employees at Texas Children’s unless approved for a medical or religious exemption
- Required Bachelor of Science degree in Pharmacy with equivalent experience or clinically oriented Master of Science degree accepted; Doctorate of Pharmacy degree preferred
- Required RPH - Pharmacist by the State of Texas
- Required PGY1 ASHP-accredited Pharmacy Practice Residency AND a PGY2 Pharmacy Practice Residency in an Advanced Area of Pharmacy Practice, or
- Required PGY1 ASHP-accredited Pharmacy Practice Residency with an additional four (4) years’ experience in a pediatric inpatient hospital pharmacy setting with clinical responsibilities
