We are searching for a Care Coordination Assistant -- someone who works well in a fast-paced setting. In this position, you will provide non-clinical administrative support to the Care Coordination process including but not restricted to timely discharge planning and utilization management.

Qualifications
Being fully vaccinated against COVID-19, including any booster dose(s) of the COVID-19 vaccine recommended by the Centers for Disease Control when eligible, is required for all employees at Texas Children’s, unless approved for a medical or religious exemption
• High school diploma or GED required
• Some college course work preferred
• 2 years’ experience in hospital care coordination/case management, home health, quality improvement or a related field required
Responsibilities
• Provides department-related and other appropriate communications to internal and external customers
• Provides professional, efficient, and courteous service to the customer while maintaining integrity in dealing with issues of a confidential nature
• Receives initial / concurrent review requests, faxes updated review to payer (if available) and refers outstanding issues to Care Coordinator
• Provides assistance for utilization management processes to maintain compliance with federal, state and managed care guidelines
• Provides assistance for transition/discharge planning and care coordination processes to transition plans and enhance patient/family satisfaction
• Assists with coordination of DME delivery, home care services, and other discharge planning activities under direction of the Care Coordinator
• Assists Care Coordinators with discharge referrals by providing initial and follow-up phone calls to providers, assisting with paperwork completion and copying/faxing pertinent documentation as directed
• Performs data entry of information obtained from Care Coordinators and payers
