We are searching for a Care Coordination Assistant for Program Operations -- someone who works well in a fast-paced setting. In this position, you will provide non-clinical administrative support to the Care Coordination process.

Responsibilities :

  • Provides updated case management database and statistics monthly and quarterly. Submits data collection in a timely manner monthly.
  • Provides assistance in telephonic outreach and data collection through special disease management projects that are grant based.
  • Ensures that projects assigned are completed timely and accurately to support member population assessments and appointments
  • Gathers data for statistics by product, service, and network and maintains all required logs
  • Assists internal and external departments by providing communication and education regarding case management and transition services and processes
  • General business and computer knowledge, medical terminology training, some college course work, and experience in health care/insurance environment are also preferred
  • Excellent written and verbal communication skills required

  • High school diploma or GED required; Associate degree or higher preferred
  • Two years’ experience in Case management, community outreach, healthcare, and/or health insurance experience OR three years’ general administrative experience required
  • Two years’ experience call/contact center experience preferred
  • Intermediate level MS Office (Word, Excel, Teams and Outlook) preferred
