We are searching for an HIM Document Specialist -- someone who works well in a fast-paced setting. In this position, you will ensure a complete recorded history of an episode of care in the patient medical record by performing quantitative analysis of the electronic health records.

Responsibilities :

Job Duties & Responsibilities

  • Analyze medical records for completeness and accuracy of documentation in accordance with regulatory standards, medical staff by-laws, rules and regulations for accuracy within 5 days of discharge
  • Analyze medical records for deficiencies in accordance with JCAHO standards, medical staff by-laws, rules and regulations for accuracy
  • Follow the department analysis guidelines.
  • Interpret notes to determine whether there are deficiencies in medical records
  • Determine appropriate physicians to complete deficiencies in the medical records
  • Assist others in resolving problem charts
  • Enter deficiencies into the Electronic Health Records
  • Re-analysis of Done/Declined records

Skills & Requirements

  • Associate’s degree in health information management (HIM), healthcare administration, or business required
  • 3 years’ experience working with electronic records required
  • RHIA or RHIT certification preferred
  • Bachelor's degree may substitute for 2 yrs experience
